This ancient Temple is situated at a distance of 25 Km from Banswara City in Village Paraheada of Tehsil Garhi, District Banswara, Rajasthan. On the Banswara-Garhi road, one has to cover a distance of about 17 Km to reach Talwara, then go via Samagada and take left turn at Samagada. The Temple is just beyond the Village Kusalpur.

This Temple was built in the 12th Century A.D. by King Mandalik. It is situated on the bank of Nagela Tank.
